By signing this permission / consent form you are allowing French Revolver Studio to supply you with a chemical service to use on yourself.

It is extremely important that you make French Revolver Studio aware of any and all processes you have used on your hair in the past months while in lockdown. The following things can have an effect on the hair:

• Boxed colours / dyes

• Porous bleached hair if bleached more than 2 x in a month

• Use of henna

• Metal based colours, e.g supermarket range. “Just for Men”

Liability Waiver

I have been made aware that my desired outcome may vary.

I am aware of all the processes and procedures I have used on my hair in the past few months may alter the process that I need to achieve my desired results. I realise this is very important information and that any information withheld regarding my previous processes will increase my chance of damage and the potential for unpredictable chemical reactions.

I understand that these are extraordinary times, and this is an extraordinary measure. I am agreeing to apply my own colour because of this.

I understand that the sale of this lockdown colour kit is final. Any corrections needed are my own expense.
